Alpha - tocopherol acetate (vitamin E)

General characteristics. Structure:

Active ingredient: 0,1 g of alpha tocopherol of acetate (vitamin E).

Excipient: sunflower oil.

Structure of a cover: glycerin, Natrium benzoicum, the water purified gelatin.

Pharmacological properties:

Pharmacodynamics. Vitamin E is antioxidant means, participates in processes of fabric metabolism, prevents hemolysis of erythrocytes, increase in permeability and fragility of capillaries, dysfunction of seed tubules and testicles, placentae, normalizes reproductive function.

Drug interferes with development of atherosclerosis, degenerative and dystrophic changes in a cardiac muscle and skeletal muscles, improves food and sokratitelny ability of a myocardium, reduces consumption by an oxygen myocardium. Slows down free radical reactions, prevents formation of the peroxides damaging cellular and subcellular membranes. Stimulates synthesis gem and gemsoderzhashchy enzymes - hemoglobin, a myoglobin, tsitokhrom, a catalase, peroxidase.

Improves tissue respiration, stimulates protein synthesis (collagen, fermental, structural and sokratitelny proteins of skeletal and unstriated muscles, a myocardium), protects vitamin A from oxidation, inhibits cholesterol synthesis. 

In the absence of vitamin E in an organism (avitaminosis E) degenerative changes in skeletal muscles of heart develop, permeability and fragility of capillaries increases, the epithelium of seed tubules, testicles regenerates. Embryos have hemorrhages, there occurs their pre-natal death. Also degenerative changes in nervous cells and defeat of a parenchyma of a liver are established.

At insufficiency of vitamin E decrease in concentration of proteins in blood serum and contents of nucleic acids in a liver and seed plants is observed.

Insufficiency of vitamin E at the person can be connected with feeding habits is caused by various diseases, for example, of a liver, a pancreas, etc.

Pharmacokinetics. From a GIT about 50% of the entered dose are soaked up, the maximum level in blood is created in 4 hours. Absorption requires availability of bile acids. In the course of absorption forms a complex with the lipoproteids which are intracellular carriers of vitamin E. Comes, mainly, to a lymph, then – to the general blood stream where contacts generally with alfa-1 and beta lipoproteids, partially a seralbumin. At disturbance of exchange of proteins transport is at a loss. It is deposited in adrenal glands, a hypophysis, seed plants, fatty and muscular tissue, erythrocytes, a liver. Is exposed to metabolism and it is allocated from an organism with bile (over 90%) and urine (about 6%) in not changed look and in the form of metabolites.

Indications to use:

Apply at a hypovitaminosis, reconvalescence after the diseases proceeding with a feverish syndrome, an asthenic and neurotic syndrome, degenerative changes of the copular device (including a backbone), joints, muscles; dysfunction of gonads at men; women – at threat have abortions and various disturbances of a menstrual cycle, at deterioration in conditions of pre-natal fetation.

Drug is shown at muscular dystrophies, an amyotrophic side sclerosis, psoriasis, a dermatomyositis, etc. collagenoses.

Vitamin E is applied also at defeats of peripheral vessels (an endarteritis, a Raynaud's disease), a myocardial dystrophy, atherosclerosis, spasms of peripheral vessels, an idiopathic hypertensia, cardiovascular insufficiency.

Appoint vitamin E and at seborrhea, atrophic changes of skin (a scleroderma, etc.), trophic ulcers of a shin, photodermatoses.

Route of administration and doses:

At disturbance of a spermatogenesis and potentiality to men appoint 100-300 mg of an - acetate tocopherol in days (in combination with hormonal therapy) within a month; at the menacing abortion – on 100-150 mg a day within 7-14 days; at primary abortion and deterioration in conditions of pre-natal fetation – on 100-150 mg within the first 2-3 months of pregnancy daily or every other day. At muscular dystrophies, an amyotrophic side sclerosis, dermatomyositis, etc. collagenoses vitamin E is applied on 15-100 mg a day within 1-2 months; repeated courses are appointed in 2-3 months; at diseases of peripheral vessels, a myocardial dystrophy, atherosclerosis – on 100 mg a day (in combination with vitamin A) within 20-40 days, in 3-6 months the course of treatment can be repeated.

At diseases of skin appoint drug from 15 to 100 mg a day within 20-40 days.

Features of use:

With care it is necessary to apply at a heavy cardiosclerosis, a myocardial infarction, at the increased risk of development of thromboembolisms.

Drug is not used in pediatric practice because of a high dosage of tocopherol of acetate in one capsule.

Pregnancy. Vitamin E is applied at pregnancy according to indications. Vitamin E gets through a placenta in insufficient quantities: in blood of a fruit 20-30% of concentration get into mother's blood.

Lactation period. Use of drug in the period of a lactation is carried out only under observation and according to the recommendation of the doctor.

Features of influence of medicine on ability to manage the vehicle or potentially dangerous mechanisms. Does not influence.

Side effects:

Allergic reactions. At reception of high doses – diarrhea, pains in epigastriums. With care it is necessary to apply at a heavy cardiosclerosis, a myocardial infarction, at the increased risk of development of thromboembolisms.

Interaction with other medicines:

Increases efficiency of anticonvulsants at patients with epilepsy at whom the increased content in blood of products of peroxide oxidation of lipids is revealed.

Strengthens effect steroid and NPVS, cardiac glycosides, reduces toxicity of the last, and also vitamins A and D.

Contraindications:

Hypersensitivity to drug, a cardiosclerosis, a myocardial infarction.

Overdose:

Symptoms: at reception during the long period in doses of 400-800 ME/days – an illegibility of visual perception, dizziness, a headache, nausea, diarrhea, a gastralgia, an adynamy; at reception more than 800 ME/days during the long period – increase in risk of development of bleedings in patients with a hypovitaminosis To, disturbance of metabolism of thyroid hormones, disorder of sexual function, thrombophlebitis, a thrombembolia, necrotic colitis, sepsis, a hepatomegalia, a hyperbilirubinemia, a renal failure, hemorrhages in a mesh cover of an eye, a hemorrhagic stroke, ascites, hemolysis.

Treatment: drug withdrawal; appoint the glucocorticoids accelerating metabolism of vitamin E in a liver; appoint Vikasolum for reduction of danger of hemorrhages.

Storage conditions:

To store in the place protected from moisture and light at a temperature from 15 ºС to 25 ºС. To store in the place, unavailable to children! Period of validity 2 years.
