General characteristics. Structure:

Active ingredient: paracetamol; 1 tablet contains paracetamol 500 mg.

Excipients: cellulose microcrystallic, sodium of a kroskarmelloz, magnesium stearate, K 90 povidone, talc, Opadry 31G58920 covering white: polyethyleneglycol; gipromeloza; lactose, monohydrate; titanium dioxide (Е 171); talc.

Pharmacological properties:

Paracetamol – non-narcotic analgetik-antipyretic which analgetichesky effect is caused by the central and peripheral action. Paracetamol raises a threshold of painful sensitivity, blocks bradikininchuvstvitelny receptors, has weak antiinflammatory effect owing to oppression of synthesis of E2 and F2 prostaglandins by inhibition of TsOG (cyclooxygenase).
The febrifugal effect of paracetamol is connected with oppression of biosynthesis of prostaglandins which are mediators of the center of thermal control in a hypothalamus.
Paracetamol effectively inhibits TsOG in cells of the central nervous system and is much weaker – in peripheral fabrics.

Drug is quickly soaked up in digestive tract, gets into all body tissues, contacting proteins of a blood plasma. The maximum concentration in a blood plasma is observed in 30-40 min. after intake. The febrifugal effect occurs in 2 hours. Action duration – 3–4 hours. An elimination half-life – 1–4 hours. It is metabolized in a liver with formation of glucuronides and sulfates of paracetamol. It is removed by kidneys, mainly in the form of products of conjugation, less than 5% are excreted in not changed look.

Indications to use:

Pain syndrome of weak or moderate extent of any genesis (a headache, a dentagra, neuralgia, a myalgia, arthralgias, primary dysmenorrhea), fever and a fever at cold, flu.

Route of administration and doses:

Tablets apply inside, after food, washing down with water. Depending on disease severity to adults and children 12 years are more senior apply 500–1000 mg (1–2 tablets) 2–4 times a day with an interval between receptions not less than 4 hours. No more than 8 tablets for 24 hours.  
The maximum daily dose for adults – 4 g.
The course of treatment makes from 3 to 5 days; under conditions of reception of higher doses – 3 days.

Features of use:

Use during pregnancy or feeding by a breast.
During the periods of pregnancy and feeding a breast drug is appointed with care if the expected advantage for mother exceeds potential risk for a fruit or the child. In that case drug is appointed only after consultation with the doctor in a minimal effective dose during no more than 3 days.

Ability to influence speed of response at control of motor transport or work with other mechanisms.
There are no data on negative influence of paracetamol on ability to drive vehicles and to work with difficult mechanisms.

Children.
To children aged up to 12 years paracetamol is appointed in other dosage form.

Not to exceed the recommended dose. Not to exceed duration of a course of treatment. If expressiveness of symptoms of a disease does not decrease within 3 days of treatment by drug, repeated consultation of the doctor is necessary.

Side effects:

- From the alimentary system: nausea, an abdominal pain, an abnormal liver function are possible.
- Allergic reactions: skin rash, an itch, a small tortoiseshell, a Quincke's disease are possible.
- From an urinary system: seldom, at prolonged use – a renal failure, an aseptic pyuria.
- From system of a krovoobrazovaniye: at prolonged use – anemia, an agranulocytosis, thrombocytopenia, a methemoglobinemia.
- From a nervous system: seldom – drowsiness or superexcitation.

Interaction with other medicines:

It is not necessary to appoint drug along with other means which contain paracetamol. Speed of absorption of paracetamol can increase at a concomitant use of Metoclopramidum and a domperidon, to decrease – at a concomitant use of Colestyraminum. Paracetamol can strengthen anticoagulating effect of warfarin and other derivatives of coumarin. Barbiturates reduce febrifugal effect of paracetamol.
At a concomitant use of barbiturates, tricyclic antidepressants, rifampicin, Butadionum the risk of a hepatotoxic action of drug increases.
At a concomitant use with salicylates the risk of nephrotoxic action considerably increases.

Contraindications:

Hypersensitivity to paracetamol. Diseases of a liver and kidneys, diabetes mellitus, blood diseases, fenilketonuriya, genetic deficit glyukozo-6-fosfatdegidrogenazy.
І pregnancy trimester.

Overdose:

The overdose of paracetamol can cause development of a liver failure.
Symptoms: pallor of skin, lack of appetite, nausea, vomiting, an abdominal pain, increase in activity of liver enzymes, increase in a prothrombin time, appears pain in a liver later. In hard cases the liver failure, coma and encephalopathy develops.
Treatment: a gastric lavage within the first hours after administration of drug in an overdose. Reception of enterosorbents, intravenous administration of an antidote of N-Acetylcysteinum, appointment in methionine.

Storage conditions:

To store in the place protected from light at a temperature not above 25 °C.
To store in the place, unavailable to children.
