General characteristics. Structure:
Active ingredient: 500 mg or 1000 mg of cefazolin of sodium salt.
The Tsefalosporinovy antibiotic of a broad spectrum of activity having bactericidal effect.
Pharmacological properties:
Pharmacodynamics. Cefazolin sodium salt — a tsefalosporinovy antibiotic of a broad spectrum of activity. Has bactericidal effect. It is active concerning gram-positive bacteria: Staphylococcus spp. (the strains which are producing and not producing a penicillinase), Streptococcus spp. (including Streptococcus pneumoniae), Corynebacterium diphtheriae, Bacillus anthracis; gram-negative bacteria: Neisseria meningitidis, Neisseria gonorrhoae, Shigella spp., Salmonella spp., Escherichia coli, Klebsiella spp. Leptospira spp is active also concerning Spirochaetaceae. The indole - positive strains of Proteus spp is inactive concerning Pseudomonas aeruginosa., Mycobacterium tuberculosis, anaerobic bacteria.
Reacts with specific penitsillinsvyazyvayushchy proteins to cytoplasmic membrane surfaces, slows down synthesis of a peptidoglikanovy layer of a cell wall (inhibits transpeptidase, formation of cross stitchings of chains of a peptidoglikan oppresses), releases autolytic enzymes of a cell wall, causing its damage and death of bacteria.
Pharmacokinetics. Cefazolin sodium salt collapses in digestive tract.
After parenteral administration in blood high concentration of drug are created. The elimination half-life (T1/2) makes 1,8 — 2 hours. At patients with renal failures of T1/2 value increase. Cefazolin gets into a bone tissue, into ascitic liquid, pleural and synovial liquids, bile; easily passes through a placental barrier, however it is not capable to get through a blood-brain barrier. It is removed by kidneys in not changed look (for 24 h from 80 to 100% of the entered dose are removed), in insignificant quantities it is removed with breast milk.
Indications to use:
Cefazolin is applied at the infectious and inflammatory diseases caused by microorganisms, sensitive to it.
Indications to use of drug are: infections of upper and lower parts of respiratory tracts (the acute bronchitis and an exacerbation of chronic bronchitis infected bronchiectasias, bacterial pneumonia, an empyema and abscesses of a lung); infections of ENT organs (including average otitis, mastoiditis); infections of urinary tract (including acute and chronic pyelonephritis); infections of biliary tract; infections of skin and soft tissues (including cellulitis, erysipelatous inflammation, anthrax, phlegmon, wound and burn fevers); infections of bones and joints (including osteomyelitis, septic arthritis); gynecologic and puerperal infections (including septic abortion, endometritis, puerperal infections); septicaemia, endocarditis, peritonitis, syphilis, gonorrhea; prevention of postoperative infectious complications.
Route of administration and doses:
The mode of dosing is set individually. The daily dose of cefazolin makes for adults from 1 to 4 g (sometimes more) and depends on weight of an infection, a type of the activator and its sensitivity to an antibiotic. If infections are caused by gram-positive microorganisms, the single dose of cefazolin for adults makes usually 0,25 — 0,5 g, the drug is administered by each 8 h.
At the moderately severe respiratory infections caused by pneumococci at diseases of the urinary tract caused by microorganisms, sensitive to an antibiotic, each 12 h appoint 0,5 — 1,0 g.
At the diseases caused by gram-negative microorganisms, sensitive to an antibiotic, each 6 — 8 h appoint 0,5 — 1,0 g of cefazolin.
Daily dose at sepsis, an endocarditis and other serious illness — to 6 g.
The maximum daily dose of drug — 6 g. Treatment duration — 7 — 10 days.
For prevention of postoperative infections enter in a dose 1 g in 30 min. prior to operation, 0,5 — 1,0 g — during operation and on 0,5 — 1,0 g — each 6 — 8 h within a day after operation.
The patient with disturbances of secretory function of kidneys of cefazolin sodium salt is appointed in the reduced doses in order to avoid cumulation.
Features of use:
At emergence of the heavy diarrhea characteristic of pseudomembranous colitis, cefazolin sodium salt it is necessary to cancel and appoint the corresponding treatment. During treatment patients should abstain from alcohol intake.
At prolonged use of drug control of a picture of peripheral blood and function of kidneys is necessary; it is necessary to perform prevention of dysbacteriosis.
With care appoint drug at abnormal liver functions and kidneys, prick, decrease in coagulability of blood. With extra care it is applied along with loopback diuretics.
Side effects:
Nausea, vomiting, diarrhea, pains in epigastric area, pseudomembranous colitis, increase in concentration of liver enzymes (nuclear heating plant, ALT, an alkaline phosphatase, LDG) in blood, a neutropenia, a leukopenia, an agranulocytosis, thrombocytopenia, a lymphocytosis, hemolitic anemia, a candidiasis, allergic manifestations (skin rashes, an itch, a toxic epidermal necrosis, Stephens-Johnson's syndrome, a mnogoformny erythema, a Quincke's disease, an acute anaphylaxis), reactions in an injection site: formation of infiltrates and abscesses, phlebitis and thrombophlebitises.
Interaction with other medicines:
Probenetsid slows down excretion of cefazolin of sodium salt and promotes cumulation.
It is impossible to mix cefazolin sodium salt rolled into one or the syringe with other antibiotics (especially with antibiotics of group of aminoglycosides).
At values рН solution more than 8,5 cefazolins sodium salt is exposed to hydrolysis, at рН lower than 4,5 the deposit (insoluble cefazolin) is formed.
Contraindications:
Hypersensitivity, including to others beta лактамным to antibiotics, pregnancy, feeding by a breast (for the period of treatment stop).
Safety of drug at children aged up to 1 month and at premature children is not established therefore use of cefazolin of sodium salt at this category of patients is not recommended.
Overdose:
Symptoms: drowsiness, paresthesias, a headache, are possible spasms, especially at patients with a renal failure. Changes of laboratory indicators at overdose: increase in level of creatinine in blood serum, change of functional hepatic tests, positive test of Koombs, a thrombocytosis, thrombocytopenia, an eosinophilia, a leukopenia, lengthening of a prothrombin time.
Treatment: symptomatic therapy. At emergence of convulsive attacks anticonvulsant therapy is performed. In case of heavy overdose, especially at patients with a renal failure, apply a hemodialysis and hemosorption.
