Пилобакт

Пилобакт – antiulcerous drug which has bactericidal effect on Helikobakter of a pilora (the spiral-shaped gram-negative bacterium living only in a stomach of the person).

Form of release and structure

Пилобакт release in the form of the combined set of tablets and capsules (2 capsules of an omeprazol, 2 tablets of a klaritromitsin and 2 tablets of Tinidazolum (only 6 pieces) in blisters, on 7 blisters in a cardboard pack).

Active agent is a part of 1 capsule of an omeprazol: омепразол – 20 mg.

In structure of 1 tablet, coated, the klaritromitsina enters active agent: кларитромицин – 250 mg.

The structure of 1 tablet, coated, Tinidazolum includes active agent: Tinidazolum – 500 mg.

Indications to use

Пилобакт appoint for eradikatsionny therapy Helikobakter of a pilora (Helicobacter pylori) at a peptic ulcer of a duodenum.

Contraindications

• Porphyria;
• Oppression of a marrowy hemopoiesis;
• Liver/renal failure;
• Organic diseases of the central nervous system;
• Simultaneous use with ethanol, terfenadiny, tsizapridy, astemizoly, Pimozidum;
• Pregnancy and period of a lactation;
• Age up to 18 years;
• Hypersensitivity to an omeprazol, Tinidazolum or a klaritromitsin, and also to antibiotic drugs of group of macroleads.

Route of administration and dosage

Пилобакт accept inside, it is desirable – during meal or after food. Tablets and capsules need to be swallowed entirely, without breaking and without chewing them.

The single dose of Pilobakt makes:

• Omeprazol – 1 capsule;
• Klaritromitsin – 1 tablet;
• Tinidazolum – 1 tablet.

Frequency rate of administration of drug – 2 times a day (in morning and evening).

Duration of a medical course – 7 days.

Side effects

• Alimentary system: dryness in a mouth, a meteorism, dysbacteriosis, a lock or diarrhea, vomiting, metal smack in a mouth, stomatitis, nausea, an abdominal pain, taste disturbance, a loss of appetite, tranzitorny increase in activity of liver enzymes in plasma, functional disturbances of a liver, hepatitis (including with jaundice), encephalopathy at liver diseases in the anamnesis; seldom – a pseudomembranous coloenteritis;
• Central nervous system: sonitus, ataxy, headache, excitement, dizziness, increased fatigue, drowsiness, weakness, sleeplessness, depression, paresthesias, confusion of consciousness, epileptic reactions, hallucinations, disorientation, peripheral neuropathy, depersonalization, fear, spasms, dreadful dreams, psychosis;
• Musculoskeletal system: mialgiya, muscular weakness, arthralgia;
• Hemopoietic system: Werlhof's disease, leukopenia, thrombocytopenia, neutropenia, anemia;
• Integuments: itch; seldom – skin rash; very seldom – a multiformny exudative erythema, a photosensitization, an alopecia;
• Allergic reactions: acute anaphylaxis, Quincke's disease, small tortoiseshell, bronchospasm;
• Others: fever, sweating strengthening, tachycardia, vision disorder, intersticial nephrite, peripheral hypostases, gynecomastia.

Special instructions

Prior to the beginning of a medical course it is necessary to exclude malignant process (masking of symptomatology can complicate statement of the right diagnosis).

It is necessary to be careful at combined use with the drugs which are metabolized a liver. At the combined use with warfarin or other indirect anticoagulants it is necessary to exercise control of a prothrombin time.

In the presence of instructions in the anamnesis on heart troubles the concomitant use with tsizapridy, terfenadiny, astemizoly is not recommended.

Tinidazolum can cause coloring of urine in dark color.

During therapy it is necessary to be careful at control of motor transport and performance of other potentially dangerous types of works which demand high concentration of attention and bystry psychomotor reactions.

Medicinal interaction

At the combined use of the medicines which are a part of set with some drugs / substances the following effects can be observed:

• Theophylline (with klaritromitsiny): increase in its concentration;
• Zidovudine (with klaritromitsiny): decrease in its absorption (the break between receptions is necessary not less than 4 hours);
• Lincomycin, clindamycin (with klaritromitsiny): development of cross resistance;
• Terfenadin (with klaritromitsiny): increase in its concentration and, as a result, lengthening of an interval of QT;
• Indirect anticoagulants (with klaritromitsiny): potentiation of their action;
• Carbamazepine, cyclosporine, Phenytoinum, Disopyramidum, ловастатин, Valproatum, цизаприд, Pimozidum, астемизол, digoxin (with klaritromitsiny): increase in their level is possible;
• Phenytoinum, diazepam, warfarin (with omeprazoly): delay of their elimination is probable;
• Ketokonazol, ampicillin and salts of iron (with omeprazoly): exerts impact on their absorption;
• Indirect anticoagulants, ethanol (with Tinidazolum): there is strengthening of their action (disulfiramopodobny reactions can be observed);
• Phenobarbital (with Tinidazolum): there is an acceleration of metabolism of Tinidazolum.

Omeprazol strengthens an inhibiting effect on the hemopoietic system of other medicines.

Simultaneous use of Tinidazolum with Etioniamidum is not recommended.

Terms and storage conditions

To store in the place, dry, unavailable to children, at a temperature up to 25 °C.

Period of validity – 3 years.
