Назонекс – local synthetic drug with antiinflammatory and antiallergic action.

Form of release and structure

Назонекс is issued in the form of spray of nasal dosed: almost white or white color (in polyethylene bottles on 10 g (60 doses) or 18 g (120 doses), on 1-3 bottles in a cardboard pack with the portioning device in a set).

Is a part of 1000 mg of spray:

• Active ingredient: furoate of a mometazon (in the form of monohydrate, micronized) – 0,5 mg;
• Auxiliary components: monohydrate of citric acid – 2 mg, the microcrystallic cellulose processed karmellozy sodium (disperse cellulose) – 20 mg, глицерол – 21 mg, polysorbate of 80 - 0,1 mg, a sodium citrate dihydrate – 2,8 mg, chloride a benzalkoniya (50% solution) – 0,2 mg, the purified water – 950 mg.

Indications to use

• Allergic rhinitis (year-round, seasonal) at children of 2 years and adults (treatment);
• Sinusitis (acute current) or exacerbation of chronic sinusitis at children of 12 years and adults, including elderly patients (as auxiliary drug against the background of treatment by antibiotic medicines);
• Rinosinusit (an acute current) with easily and moderately expressed symptoms in the absence of displays of a heavy bacterial infection at children of 12 years and adults;
• Seasonal allergic rhinitis (heavy and medium-weight current) at children of 12 years and adults (as prophylactic). It is recommended to begin Nazoneks's use in 14-28 days prior to the estimated beginning of a season of dusting;
• Nose polyposes, proceeding with disturbances of nasal breath and sense of smell, at adults.

Contraindications

Absolute:

• Nose injuries with injuries of a mucous membrane of a nasal cavity or the operative measure postponed in recent time (Nazoneks's use perhaps after a wound repair that is connected with influence of drug on healing processes);
• Age up to 2 years (at treatment of year-round and seasonal allergic rhinitis), up to 12 years (at treatment of an exacerbation of chronic sinusitis, acute sinusitis) or up to 18 years (at treatment a polypose) that is connected with insufficiency of data on efficiency and safety of use of Nazoneks for these age groups of patients;
• Hypersensitivity to drug components.

Relative (Nazoneks appoint with care in the presence of the following states / diseases):

• Tuberculosis infection of a respiratory path (active or latent);
• Not treated system viral, bacterial, fungal infection or infections caused by a herpes simplex (Herpes simplex) with damage of eyes (as the exception of Nazoneks at the listed infections can be applied according to instructions of the doctor);
• Not treated local infections proceeding with involvement in process of a mucous membrane of a nasal cavity.

To pregnant women and the feeding women Nazoneks's appointment only after ratio assessment advantage/risk for health of mother and the child is possible.

Route of administration and dosage

Назонекс it is necessary to apply intranazalno, 1 dose makes 0,05 mg.

The scheme of use of spray is defined by indications:

• Seasonal or year-round allergic rhinitis: the recommended daily dose (therapeutic and preventive) for children of 12 years and adults makes 2 inhalations in each nostril (totally – 0,2 mg). Drug is used once a day. If necessary increase in a dose up to 4 inhalations in each nostril is possible. After improvement of a state for performing the supporting treatment reduction of an average therapeutic dose is recommended twice. To children of 2-11 years, as a rule, appoint in each nostril 1 inhalation once a day. Improvement is usually noted in 12 hours from the moment of the first use of Nazoneks;
• Acute sinusitis, exacerbation of chronic sinusitis (auxiliary therapy): to children of 12 years and the adult, including elderly patients, appoint 2 times a day 2 inhalations in each nostril (totally – 0,4 mg a day). If it is not possible to reach reduction of expressiveness of symptoms, the single dose can be increased twice. After improvement of a state it is lowered to therapeutic;
• Acute rinosinusit without symptoms of a heavy bacterial infection: the adult appoint 2 inhalations 2 times a day (totally – 0,4 mg a day). If improvement is not noted, it is necessary to consult with the doctor about expediency of further use of Nazoneks;
• Nose polyposes: the adult, including elderly patients, appoint 2 inhalations 2 times a day (totally – 0,4 mg a day). After improvement of a state the frequency of use of spray is reduced twice.

Before the first use of Nazoneks it is necessary to carry out calibration for what it is necessary to press up to 10 times the portioning device. Emergence of splashes will testify to readiness of spray for use. If having rummaged in use of drug makes more than 14 days, before holding a procedure it is necessary to press the dosing nozzle several times (before emergence of splashes).

Before use the bottle with spray needs to be stirred up vigorously. Назонекс enter, having inclined the head.

For ensuring the correct work of the dosing nozzle, it needs to be cleaned regularly for what at first it is necessary to remove a cap which protects a nozzle, then a spray hand piece. The tip needs to be washed out carefully in warm water, then to rinse under flowing water.

To open the nasal applicator, using for this purpose a needle or other sharp object, it is impossible as it can lead to its damage and the wrong drug dosing.

Side effects

During Nazoneks's use depending on age development of the following disturbances is possible:

• Children: a headache, nasal bleedings, sneezing, feeling of irritation in a nose;
• Teenagers and adults: burning sensation in a nose, an ulceration of a mucous membrane of a nose, irritation of a mucous membrane of a nasal cavity, pharyngitis, a headache, nasal bleedings (are shown as explicit bleedings or allocations by the painted blood of slime or clots). Nasal bleedings usually have short-term, not character.

At Nazoneks's use by patients of all age groups the following side effects can develop:

• Seldom: hypersensitivity reactions of immediate type, including a bronchospasm, an asthma;
• Very seldom: increase in intraocular pressure, anaphylaxis, Quincke's disease, disturbances of sense of smell and taste, perforation of a nasal partition.

Special instructions

When performing long-term treatment by Nazoneks it is necessary to perform periodically inspections of a mucous membrane of a nose at the ENT specialist. At development of local fungal infections of a nose or throat it is necessary to interrupt therapy and to begin performing special treatment. The long irritation of a mucous membrane of a pharyngeal cavity and a nose can become the reason for drug withdrawal.

At prolonged use of spray of signs of suppression of function гипоталамо - the pituitary and adrenal system was not noted.

The patient passing to Nazoneks's use after long treatment with system glucocorticosteroids requires special attention as their cancellation can result in insufficiency of function of adrenal glands. For its recovery several months can be required. In cases of development of symptoms of adrenal insufficiency it is necessary to resume therapy by system glucocorticosteroids and to take other necessary measures.

Also at some patients at implementation of such transition development of symptoms of cancellation of system glucocorticosteroids (feelings of fatigue, joint and/or muscular pains are usually shown in the form of a depression), despite decrease in expressiveness of the signs connected with damage of a mucous membrane of a nose is possible. In such cases it is reasonable to continue therapy by Nazoneks. Change of the scheme of treatment can sometimes reveal the developed earlier allergic diseases, such as eczema and allergic conjunctivitis which were not shown because of therapy by system glucocorticosteroids before.

After therapy of GKS patients because of decrease in immune responsiveness have a high risk of infection at contact with patients, including measles and chicken pox. If such contact happened, it is necessary to notify on it the doctor.

In case of symptoms of the expressed bacterial infection (sharp and persistent pain on the one hand of the person, fevers, a dentagra, a swelling in orbital or periorbital area) needs to consult immediately at the doctor.

Medicinal interaction

Joint appointment of Nazoneks with loratadiny is, as a rule, transferred well. At the same time any influence of drug on concentration of a loratadin was not noted.

Terms and storage conditions

To store in the place, unavailable to children, at a temperature of 2-25 °C, not to freeze.

Period of validity – 2 years.
