Aprovel

Aprovel – a synthetic medicine with hypotensive action.

Form of release and structure

Aprovel release in the form of tablets: biconvex, oval, film coated (in blisters on 14 pieces)

Active ingredient: ирбесартан – 150 mg and 300 mg.

Excipients: lactoses monohydrate, MKTs, sodium of a kroskarmelloz, magnesium stearate, silicon dioxide colloid, gipromelloza.

Indications to use

• Arterial hypertension (as monotherapy or in combination with other hypotensive drugs, for example, beta adrenoblockers, thiazide diuretics, BKK);
• Nephropathy at a diabetes mellitus 2 types and arterial hypertension (as a part of the combined hypotensive treatment).

Contraindications

• Hypersensitivity to active agent (irbesartan) or auxiliary components of tablets;
• Pregnancy and feeding by a breast;
• Concomitant use with the medicines containing алискирен at patients with a diabetes mellitus or moderately expressed and heavy renal failure;
• Hereditary intolerance of a galactose, insufficiency of lactase or disturbance of absorption of glucose and galactose;
• Children's age up to 18 years;
• The age is more senior than 75 years (because of impossibility of division of tablets).

Aprovel's reception is contraindicated to the patients who is on a hemodialysis.

Aprovel take with caution:

• Stenosis of the aortal or mitral valve or hypertrophic subaortic stenosis (GOKMP);
• Bilateral stenosis of renal arteries, unilateral stenosis of an artery of the only functioning kidney, chronic heart failure of the III-IV functional class;
• The hypovolemia, hyponatremia arising at therapy by diuretics, observance of a diet with restriction of consumption of table salt, diarrhea, vomiting (because of danger of an excessive lowering of arterial pressure);
• Recent transplantation of a kidney;
• Renal failure;
• Coronary heart disease and/or clinically significant atherosclerosis of vessels of a brain (the excessive lowering of arterial pressure increases risk of strengthening of ischemic disturbances);
• Heavy liver failure.

Also it is necessary to be careful at a concomitant use of Aprovel with non-steroidal anti-inflammatory drugs, including the selection TsOG-2 inhibitors (because of increase in risk of a renal failure).

Route of administration and dosage

Tablets Aprovel are swallowed entirely, washing down with water.

As a rule, the initial and maintenance dose makes 150 mg once a day that allows to provide 24-hour control of arterial pressure. At insufficient therapeutic effect the daily dose can be increased twice. In certain cases in addition appoint diuretics.

At arterial hypertension and a diabetes mellitus 2 types begin treatment with 150 mg a day, gradually increasing a dose to 300 mg.

For patients with a renal failure and a liver correction of a dose is not required.

Safety and efficiency of use of Aprovel for children is not confirmed with researches.

Side effects

At treatment of arterial hypertension the frequency and expressiveness of side effects at Aprovel's use does not depend on the appointed dose, duration of reception of a medicine, age, sex and race.

During therapy are observed:

• Often – dizziness, nausea, vomiting, fatigue;
• In some cases – tachycardia, a hyperemia of integuments, cough, diarrhea, dyspepsia, heartburn, thorax pain.

If arterial hypertension and a diabetes mellitus 2 types proceed without renal failure, at Aprovel's reception in addition to the described symptoms development of orthostatic dizziness and orthostatic hypotension is possible. If the disease is followed by a chronic renal failure and the expressed proteinuria, can be also observed:

• Often – muscle and bones pains, orthostatic dizziness, orthostatic hypotension;
• In some cases – a hyperpotassemia, a dysgeusia, a headache, a ring in ears, allergic reactions (urticaria, rash, a Quincke's disease), a mialgiya, an arthralgia, spasms, abnormal liver functions, hepatitis, a renal failure.

At overdose by Aprovel the most probable is development of the expressed lowering of arterial pressure, tachycardia and bradycardia. In this case it is necessary to establish constant observation of a condition of the patient and to carry out the supporting and symptomatic therapy.

Special instructions

At arterial hypertension without the accompanying diseases perhaps excessive lowering of arterial pressure.

At Aprovel's use for patients with a renal failure it is recommended to control periodically the level of potassium and creatinine of blood serum.
