Ekzifin

Ekzifin – antifungal drug. Active ingredient – a terbinafina a hydrochloride.

Is issued in the form of gel, cream, ointment, spray, solution for outside use, tablets for oral administration. In low concentration Ekzifin has fungicidal effect concerning dermatophytes (Trychophyton mentagrophytes, Trychophyton rubrum, Trychophyton verrucosum, Trychophyton tonsurans, Trychophyton violaceum, Epidermophyton floccosum, Microsporum canis), and also mold mushrooms (Candida albicans including). Activity concerning barmy mushrooms depending on a type of the activator has fungistatic or fungicidal character. The mechanism of action of Ekzifin consists in inhibition of a skvalenepoksidaza of membranes of cells of a mushroom, specific suppression of synthesis of sterol a fungal cell therefore there is a deficit of ergosterol, intracellular accumulation of squalene and death of a fungal cell. Drug does not influence metabolism of hormones.

Indications to Ekzifin's use

According to the instruction, Ekzifin is applied at:

• fungus diseases of skin, nails (in case of onychomycoses local forms of drug are not applied): trichophytosis, epidermophitias, microsporias, tineas, candidiasis of skin, mucous membranes;
• multi-colored herpes (local forms of Ekzifin are used);
• the heavy, widespread dermatomycoses demanding system treatment.

Contraindications to Ekzifin's use

According to the instruction to Ekzifin, this drug is contraindicated at hypersensitivity to a terbinafin, pregnancy, a lactation, aged up to 2 years, a renal, liver failure, alcoholism, endocrine pathology, oppression of a marrowy hemopoiesis, tumors, occlusal vascular pathology of extremities.

Route of administration, dosage

The dosage for the tableted Ekzifin's form makes 250 mg once a day. Duration of therapy depends on weight and a form of a disease and is defined by the doctor.

At a liver and renal failure the dosage makes 150 mg a day.

Local forms of drug are applied with a thin layer on the damaged area. Frequency rate of use – one - two times a day. Spray and solution are applied on previously cleared and dried sites.

Duration of treatment by Ekzifin at defeat of nail plates long also depends on the growth rate of nails, usually makes 1-2 months. The optimum clinical effect is reached in several months after full mycologic treatment and the termination of therapy.

At candidiases Ekzifin is applied inside only at crushing widespread defeat.

At children experience of use of Ekzifin is limited.

Side effect

When using Ekzifin's the following side effects are registered:

• dispepsichesky phenomena: nausea, loss of appetite, diarrhea, abdominal pain, feeling of overflow of a stomach);
• allergic reactions: rash, urticaria;
• musculoskeletal reactions: mialgiya, arthralgia.

Extremely seldom there is a disturbance of flavoring feelings, cholestatic jaundice, Stephens-Johnson's syndrome, a toxic epidermal necrolysis, an agranulocytosis, thrombocytopenia, anaphylactoid reactions.

If at use of drug within two weeks the clinical picture does not improve, it is necessary to define repeatedly the activator and its sensitivity to this drug. Before therapy, and also in the course of treatment it is necessary to control indicators of activity of a liver. Ekzifin's interaction with other drugs

Ekzifin reduces clearance of caffeine by 20%. The drugs inducing microsomal enzymes of a liver increase Ekzifin's clearance and analogs. Ethanol and other hepatotoxic drugs at a concomitant use with Ekzifin increase risk of hepatic dysfunction.
