Ksarelto
Application instruction:
Xarelto is an anticoagulant of direct action. Is issued in the form of tablets, film coated, with the content of active agent on 10, 15 and 20 milligrams. Active agent – ривароксабан micronized.
Pharmacological action and pharmacodynamics
According to the instruction, Ksarelto inhibits a factor Ha, and also has anticoagulating action.
The mechanism of action of Ksarelto according to the instruction consists in direct inhibition of a factor Ha. Rivaroksaban has high selectivity and has high bioavailability (80-100 percent) during intake. Activation in external and internal ways of coagulation of a factor of X with formation of a factor Ha plays the most important role in the coagulative cascade.
Pharmacokinetics
Rivaroksaban is quickly soaked up – the maximum concentration in blood is reached in two-four hours after the use of a tablet. After hit in an organism the most part of a rivaroksaban (from 92 to 95 percent) contacts plasma proteins, the main connecting component – a seralbumin.
At intake about two thirds of a dose of a rivaroksaban are exposed to metabolism and afterwards are removed in equal parts together with a stake and urine. The remained third is removed in not changed view with the help of direct renal excretion, first of all due to active secretion of kidneys.
Indications to Ksarelto's use
According to the instruction, Ksarelto in the form of tablets on 10 milligrams is shown for prevention of a venous thromboembolism at people who were exposed to big operative measures of orthopedic character on the lower extremities.
Ksarelto's tablets on 15 and 20 milligrams are shown for prevention of a system thromboembolism and a stroke at the people having fibrillation of auricles of not valve origin and also for treatment of a thromboembolism of a pulmonary artery and a deep vein thrombosis, for prevention of a recurrence of TELA and TGV.
Contraindications to Ksarelto's use
According to the instruction, Ksarelto is contraindicated at:
- Hypersensitivity to active agent (rivaroksaban) or any other substances which contain in tablets;
- Intracranial, gastrointestinal or other bleedings;
- The accompanying treatment by means of any other anticoagulants;
- States at which the high risk of big bleedings is observed;
- Pregnancy and lactic period;
- Liver diseases at which the coagulopathy is observed;
- Age up to eighteen years;
- Heavy renal failure;
- Intolerance of a galactose or lactose of hereditary character.
Side effects of Ksarelto
Reviews of Ksarelto demonstrate that use of tablets of Ksarelto can be followed by risk of the explicit or concealed hemorrhage from any fabrics or bodies which can lead to anemia of posthemorrhagic character. Also, according to responses, Ksarelto can cause hemorrhagic complications: weakness, dizziness, pallor, short wind, headache, increase in volumes of an extremity or shock.
Doses and Ksarelto route of administration
Ksarelto's pill on 10 milligrams is taken irrespective of receptions of food, on 15 and 20 milligrams – during meals.
After big operations on a knee joint duration of treatment makes two weeks, after big operations on a hip joint – five weeks. The initial dose is accepted in six-ten hours after operation if reach a hemostasis. The medical dose makes one tablet a day.
At the admission of reception of a dose it is necessary to take immediately Ksarelto's pill and to continue to accept regularly drug next day according to recommendations. For compensation of the passed dose it is forbidden to double the accepted dose.
