Zirtek

Zirtek – a blocker histamine _{H1 receptors}, antiallergic drug with antipruritic and anti-exudative action.

Form of release and structure

• Tablets, film coated: oblong, biconvex, white color, on one of the parties – risk from which on both parties the engraving of "Y" (on 7 pieces in blisters, in a pack the cardboard 1 blister is put; on 10 pieces in blisters, in a pack cardboard 1 or 2 blisters);
• Drops for intake: transparent, colourless, with a smell of acetic acid (on 10 or 20 ml in dark glass bottles droppers, in a pack a cardboard 1 bottle).

Active ingredient – a tsetirizina dihydrochloride:

• 1 tablet – 10 mg;
• 1 ml of drops – 10 mg.

Excipients of tablets: silicon dioxide colloid, lactoses monohydrate, magnesium stearate, cellulose microcrystallic, Opadray of Y-1-7000 (titanium dioxide (E171), gipromelloz (E464), macrogoal 400).

Excipients of drops: the methylparabenevils, пропилпарабензол, propylene glycol, sodium acetate, глицерол, glacial acetic acid, sodium saccharinate, the water purified.

Indications to use

• Small tortoiseshell, including chronic idiopathic;
• Pollinosis (hay fever);
• Quincke's edema;
• Seasonal and/or year-round rhinitis and conjunctivitis of allergic genesis (symptomatic treatment of such symptoms as itch, conjunctiva hyperemia, dacryagogue, sneezing, rhinorrhea);
• Other allergic dermatosis which is followed by an itch and rashes including atopic dermatitis.

Contraindications

Absolute:

• End-stage of a renal failure (clearance of creatinine less than 10 ml/minute);
• Lack of lactase, hereditary intolerance of a galactose, syndrome of glyukozo-galaktozny malabsorption;
• Children's age up to 6 months – for drops, up to 6 years – for tablets;
• Pregnancy and lactation;
• The increased individual sensitivity to drug or Hydroxyzinum.

Relative:

• Chronic renal failure;
• Chronic diseases of a liver;
• Advanced age.

Route of administration and dosage

Zirtek accept inside.

The recommended doses:

• Adults: on 10 mg (on 1 tablet or 20 drops) 1 time a day;
• Children are more senior than 6 years: on 10 mg of 1 times a day or on 5 mg (1/2 tablets or 10 drops) 2 times a day;
• Children of 2-6 years: on 2,5 mg (5 drops) 2 times a day or 5 mg (10 drops) of 1 times a day;
• Children of 1-2 years: on 2,5 mg (5 drops) 1-2 times a day;
• Children of 6-12 months: on 2,5 mg (5 drops) of 1 times a day.

In certain cases to adults and children 6 years for achievement of therapeutic effect enough 5 mg a day are more senior.

To people of advanced age and patients with a renal failure the dose is adjusted depending on the clearance of creatinine (CC) which is calculated as follows:

• For men: KK (ml/minute) = [140 - age (advanced in years)] x body weight (in kilograms)/72 x serumal creatinine (mg/dl);
• For women: KK (ml/minute) = [140 - age (advanced in years)] x body weight (in kilograms)/72 x serumal creatinine (mg/dl) x 0,85.

The recommended Zirtek's doses for patients with a liver and renal failure:

• KK of 50-79 ml/minute (slight renal failure) – 10 mg/days;
• KK of 30-49 ml/minute (average renal failure) – 5 mg/days;
• KK <30 ml/minute (a heavy renal failure) – 5 mg every other day.

At an end-stage of a renal failure drug is contraindicated.

Dose adjustment is not required to patients for whom only abnormal liver functions are diagnosed.

Side effects

Zirtek, as a rule, is well had, in rare instances there are following side effects:

• Allergic reactions: rash, urticaria, an itch, a Quincke's disease, hypersensitivity reactions, up to development of an acute anaphylaxis;
• From an organ of sight: sight illegibility, accommodation disturbance, nystagmus;
• From a metabolism: increase in body weight;
• From respiratory system: pharyngitis, rhinitis;
• From cardiovascular system: tachycardia;
• From system of a hemopoiesis: thrombocytopenia;
• From an urinary system: frustration of an urination and enuresis;
• From the alimentary system: nausea, dryness in a mouth, diarrhea, abdominal pains, an abnormal liver function (increase in level of hepatic transaminases, an alkaline phosphatase, gamma глутамилтрансферазы, bilirubin);
• From a nervous system: dizziness, drowsiness, headache, excitement, aggression, confusion, hallucinations, depression, sleeplessness, spasms, tic, dyskinesia, paresthesia, dystonia, tremor, syncope;
• Others: fatigue, indisposition, adynamy, hypostasis.

Special instructions

At Zirtek's reception in the recommended dose the negative impact on the speed of reactions and ability to concentration of attention was not revealed. Nevertheless, during treatment it is recommended to refrain from driving of vehicles and performance of potentially dangerous types of activity.

Medicinal interaction

Undesirable medicinal interaction of a tsetirizin with glipizidy, erythromycin, ketokonazoly, azithromycin, diazepam, Cimetidinum, pseudoephedrine is not revealed.

At simultaneous use of theophylline (in a daily dose of 400 mg) the general clearance of a tsetirizin decreases by 16%, the theophylline kinetics at the same time does not change.

Data on Zirtek's interaction with alcohol are not obtained, however patients are recommended to refrain from the use of alcoholic beverages during treatment by drug to avoid oppression of activity of the central nervous system.

Terms and storage conditions

To store in the place, unavailable to children, at a temperature: tablets – to 30 ºС, drops – to 25 ºС.

Period of validity – 5 years.
