Herpetiform dermatitis

Herpetiform dermatitis (dermatitis of Dyuringa, priruginozny pemphigus, polymorphic violent dermatitis) – an inflammatory disease of a mucous membrane of an oral cavity and skin. This damage of skin belongs to a chronic dermatosis of not clear etiology. Herpetiform dermatitis of Dyuringa most often develops at patients at the age of 20-45 years, and women are ill twice less often than men.

This skin disease was described for the first time by the American dermatologist L. Dyuring. After it many scientists investigated this disease, however its origin still remains to the unknown. In various researches of scientists it was not established either a virus, nor specific bacterial factor of herpetiform dermatitis. Some scientists connected development of a disease with endocrine to frustration and inherited disorders. Other researchers spoke about the allergic nature of herpetiform dermatitis as this disease proceeds against the background of disturbance of absorption in intestines and hypersensitivity of patients to the drugs containing iodine in the structure.

Symptoms and course of herpetiform dermatitis of Dyuringa

Pristupoobrazny emergence on skin and mucous bodies of the person of polymorphic pruritic rash in the form of papules, spots, bubbles, urtikarny elements, blisters is characteristic of polymorphic violent dermatitis. Rash blisters by the form remind the blisters which are formed at a burn a nettle. Bubbles in a form can be both sluggish, and dense. Contents of bubbles can be muddy, transparent or hemorrhagic. When bubbles are opened, they dry up and form honey-yellow or bloody hillocks under which further there is epithelization. At their healing on skin there is a pigmentation. Papules are usually presented by bubbles of a pryshchevidny form. All elements of rash are grouped in half rings, rings and garlands.

Burning and itch in places of damage of skin are characteristic of all forms of herpetiform dermatitis of Dyuringa. Spots remind semicircles, ringlets and deckle-edged circles. At this disease most often rash develops on the smooth surface of shoulders, hands, hips and buttocks, in sacral and scapular area, on pilar area of the head, on a neck and a face (on border of growth of hair). Disturbance of digestion because of hypersensitivity to gluten (phytalbumin of cereals) arises at nine of ten patients. The itch, burning and a pricking on externally not changed skin usually develops in 10-12 hours prior to emergence of symptoms of rash.

At children's age rash at herpetiform dermatitis is usually presented by some one element, most often bubbles. Certain patients together with skin rashes have a rash and an erosion in the form of bubbles on a mucous oral cavity.

In the absence of due treatment the course of herpetiform dermatitis can stretch for many years. The disease periodically becomes aggravated, but can forever disappear. Cases when this disease passed itself, many years later from the moment of its emergence are recorded.

At the heavy course of herpetiform dermatitis the complications arising against the background of long disturbance of digestion can affect the health of the patient.

Diagnosis and treatment of herpetiform dermatitis

In house conditions to diagnose this disease it is possible by means of iodide test. For preparation of two-percent solution of iodide potassium it is necessary to add a half of a teaspoon of iodide potassium to a glass of water. Then it is necessary to drink three tablespoons of the prepared solution at once.

It is also possible to prepare ointment from one part of vaseline and one part of iodide potassium. The received ointment should be imposed for a while in the form of a compress on skin around one of the centers. Emergence of new elements of rash will be reaction of a human body to iodine.

The dermatologist can establish an actual reason of skin rashes. The dermatologist will make the diagnosis by results of histologic researches of fabric.

The main remedy at herpetiform dermatitis are the drugs containing sulfur (sulfonamides and sulphones) in the structure. As drugs for external use means with glucocorticoids and tar in structure are usually appointed.

At digestion disturbances the doctor usually recommends to keep to an aglyutenovy diet with an exception of oats, rye, wheat, barley, millet and other cereals, and also foodstuff as a part of which there is an iodine. At addition in a diet of rice, corn, potatoes and millet within a month the broken digestion function usually is recovered.

It is possible to include regular trainings in fortifying therapy by sport, reception of vitamin complexes and various power additives.
