Dyufalak

Dyufalak – pharmacological drug on the basis of lactulose with laxative effect.

Structure and form of release

Dyufalak is issued in the form of syrup. 100 ml of medicine contain 66,7 g of lactulose and the purified water up to the volume of 100 ml.

According to the instruction Dyufalak represents transparent viscous liquid of light yellow color.

This drug is produced in plastic bottles on 200, 500 or 1000 ml (in a set – a measured glass). Also there is a release form in one-time bags from a foil and polyethylene on 15 ml (in a box of 10 packages).

Pharmacological action

This drug has laxative effect. Dyufalak possesses hyperosmotic action and by that stimulates an intestines peristaltics. Also it improves absorption of phosphates and salts of calcium, and favors to removal of ions of ammonium. Intestinal microflora of a large intestine splits lactulose to low-molecular organic acids, promotes increase in volume of contents of intestines. Thus, Dyufalak stimulates a peristaltics of intestines and influences a chair consistence. It leads to recovery of a physiological rhythm of emptying of a large intestine and elimination of locks.

Dyufalak's use at hepatic encephalopathy or a prekoma (coma) allows to reduce the content of toxic substances of a nitrogen metabolism in intestines, thereby reducing negative effect of these substances by a brain.

According to the instruction Dyufalak is prebiotic, that is strengthens growth of beneficial bacteria (bifidobacteria and lactobacilli) and suppresses growth of pathogenic flora (the Clostridium, the Salmonella) and reduces the bakteriovydeleniye period. Thus, this drug allows to provide favorable balance of an intestinal microbiocenosis.

According to Dyufalak's instruction drug has low absorption, after the use inside it reaches a large intestine in an invariable look, and there is split by microflora. In a dosage of 40-75 ml a day this medicine is completely metabolized. At reception of higher doses drug is partially removed in not changed look.

Indications to use

Dyufalak is applied at locks of a functional origin, and also to a chair softening at hemorrhoids, cracks of the proctal channel, after intestines operations and area of the proctal channel. This drug is shown at hepatic encephalopathy or a coma (prekoma).

According to the instruction Dyufalak is applied in complex treatment of dysbacteriosis, enteritis (in a bacteriocarrier stage) and a syndrome of putrefactive dyspepsia at children (as a result of acute food poisonings).

Dyufalak is applied also to preparation of a large intestine for diagnostic testings (a rektoromanoskopiya, an irrigoskopiya, a kolonoskopiya).

Contraindications to use

• Acute intestinal impassability;
• Galactosemia;
• hypersensitivity to the making drug components.

According to Dyufalak's instruction it should be used with care at patients with a lactose intolerance, at intestinal bleedings of not clear etiology. Also drug needs to be used with care at the patients with hepatic encephalopathy suffering from a diabetes mellitus.

Use during pregnancy and a lactation

According to the instruction Dyufalak does not make any impact on a fruit and on the course of pregnancy, he is allowed for use during pregnancy. Drug does not get into breast milk and can be used during feeding by a breast.

Route of administration and doses

This drug is intended for intake. Syrup can be accepted as in not divorced look, and diluted with water. If Dyufalak is appointed once a day, then it needs to be drunk at the same time (for example, in the morning during a breakfast).

At treatment of locks and for a chair softening drug is appointed by the adult in an initial dosage on 15-45 ml daily, and then pass 15-30 ml to a maintenance dose. The initial dose is selected individually, drug can once be accepted, or to be divided into 2 parts. The maintenance dose is selected depending on reaction to reception of this medicine. According to Dyufalak the therapeutic effect can appear in 2-3 days after the beginning of administration of drug. During treatment by Dyufalak it is necessary to accept enough liquid (1,5-2 liters a day).

To children the dosage of drug is selected depending on age:

• children are 7-14 years old – 10-15 ml a day;
• children are 1-6 years old of 5-10 ml a day;
• to children about one year – to 5 ml a day.

Dyufalak for newborns is also used at treatment of locks and dysbacteriosis. It is recommended to begin reception with 1 ml, gradually bringing drug volume to 4-5 ml (it will help to avoid the increased gas generation).

At treatment of hepatic encephalopathy Dyufalak's dose is selected individually, usually initial dosage makes 30-45 ml.

For preparation of a large intestine for researches 100-150 ml of drug are used. Usually the dosage depends on a type of a research, a condition of the patient and is selected the doctor individually. According to Dyufalak for these purposes it is better to use in combination with other methods of cleaning of intestines (for example, cleansing enemas).

Side effect

In the first days of use of Dyufalak emergence of the increased gas generation (meteorism) is possible. Usually it disappears in several days. At reception of high doses of drug there is diarrhea. Also there can be nausea, vomiting and abdominal pains. According to Dyufalak messages on its side effects are insufficient for exact assessment of frequency of cases.

If drug is used in the raised doses (for example, at treatment of hepatic encephalopathy), at the patient can develop disturbances of balance of electrolytes because of diarrhea.

Overdose

At reception of high doses of drug diarrhea and abdominal pains are possible. Drug withdrawal or reduction of its dose levels these manifestations. In case of disturbance of electrolytic balance correction is required by administration of electrolytic solutions.

Interaction with other medicines

Clinical trials on Dyufalak's interaction with other pharmacological drugs were not conducted.

Special instructions

In case of lack of effect of administration of drug within several days it is necessary to consult with the doctor.

It is necessary to consider that Dyufalak may contain insignificant amount of sugars (for example, lactose, a galactose, fructose). At reception of a dosage for treatment having bungled it does not represent a problem for patients with a diabetes mellitus. But at treatment of a hepatic coma (encephalopathy) the content of sugars has to be considered for this group of patients.

Patients with intolerance of a galactose, fructose should not use this drug. Dyufalak can be applied to newborns only under observation of the doctor.
