Ronkoleykin

Ronkoleykin – the drug used for providing a host defense of an organism from tumor cells and causative agents of bacterial, fungal, viral infections.

Form of release and structure

Produce drug in the form of solution for intravenous (in/in) infusions and hypodermic (п / to) introductions: colourless or light yellow transparent liquid, at a temperature of storage from 2 to 8 °C (in the refrigerator) is allowed sedimentation of the crystals of sodium lauryl sulfate which are dissolved within ½ hours at the room temperature; for acceleration of dissolution of a deposit it is allowed to incline accurately an ampoule, without mixing its contents in order to avoid foaming (on 1 ml in ampoules, on 3 or 5 ampoules in a cardboard pack).

Structure of 1 ml of solution (ME – the international units):

• Active ingredient (active substance): recombinant interleykin-2 the person (RIL-2) – 0,25, 0,5 or 1 mg ( respectively 250 000, 500 000 or 1 000 000 ME);
• Auxiliary components: Mannitolum, sodium lauryl sulfate, hydroammonium carbonate, дитиотреитол, water for injections.

Indications to use

Ronkoleykin apply along with other medicines.

At adults drug is appointed at the following diseases / states:

• The infected burns (thermal and chemical);
• Combined immunodeficience;
• Usual variable immunodeficiency;
• Osteomyelitis;
• Pancreatitis in an acute current;
• Peritonitis in an acute current;
• Pochechnokletochny cancer (in the locally-spread and disseminated forms);
• Sepsis (including sepsis after the delivery);
• Pulmonary tuberculosis;
• Heavy pneumonia;
• Endometritis;
• Others the generalized and heavy localized infections.

To children from the moment of the birth drug is appointed at treatment of such states / diseases:

• Sepsis, including bacterial sepsis of newborns;
• Combined immunodeficience;
• Usual variable immunodeficiency;
• Osteomyelitis;
• Pancreatitis in an acute current;
• Peritonitis in an acute current;
• Pneumonia in a heavy current;
• Others the generalized and heavy localized infections.

Contraindications

• Allergy to yeast;
• Autoimmune diseases;
• Pulmonary heart of the III Art.;
• Heart failure of the III Art.;
• Metastatic damage of a brain;
• Pochechnokletochny cancer in an end-stage;
• Pregnancy period;
• Hypersensitivity to drug components, including data in the anamnesis.

At a dekompensirovanny liver failure and a chronic renal failure Ronkoleykin appoint with care.

Route of administration and dosage

Ronkoleykin's solution is entered intravenously kapelno or subcutaneously.

For intravenous administration drug is transferred from an ampoule to isotonic solution of sodium chloride for injections (400 ml). Infusion of all volume is performed kapelno slowly, for 4-6 hours.

For administration of solution subcutaneously contents of 1 ampoule are parted in 1,5-2 ml of isotonic solution of sodium chloride or water for injections.

Solution before use should be examined: it has to be colourless, transparent and not contain foreign impurity.

The recommended dosing mode: 1 time a day on 0,5-1 mg with an interval from 1 to 3 days, 1-3 introductions on 1 course.

The immunotherapy should be carried out by Ronkoleykin after end of the urgent and urgent surgical interventions directed to elimination of life-threatening effects of the main illness / injury, and also sanitation and adequate drainage of the infectious center.

Drug dosage depending on a state/disease:

• Heavy sepsis (surgical, posttraumatic, obstetric and gynecologic, wound burn, etc.): from one to three courses including 2 in infusions in a dose of 0,5 mg at an interval of 1 day. The basis for purpose of the subsequent courses (second and third) is the relative and/or absolute lymphopenia remaining in the course of therapy;
• Infiltrative destructive pulmonary tuberculosis, for the first time revealed: 3 in/in infusion in a dose of 0,5 mg at an interval of the 2nd day (against the background of carrying out specific polychemotherapy);
• The progressing fibrous and cavernous tuberculosis (FCT) of lungs against the background of carrying out specific polychemotherapy (preoperative preparation): unilateral FKT – 3 in/in infusion on 1 mg at an interval of the 2nd day; widespread FKT of lungs with bilateral focal dissimination – 7 in/in infusions (3 introductions within the first week on 1 mg at an interval of the 2nd day, then on 1 mg 2 times a week within 2 weeks). The recommended course of an immunotherapy should be completed in 7-10 days prior to planned operation;
• Pochechnokletochny cancer (the disseminated and locally-spread forms): the course of therapy includes: single introduction of Ronkoleykin (in/in or п / to) in a dose of 0,5 mg in 1 days prior to operation, and also in/in administration of drug as a part of a 8 weeks course of immunochemotherapy on 2 mg every other day for the first 4 weeks of treatment (repeatedly courses of therapy are appointed in 1-2 months).

At deficit of the body weight of 30% purpose of drug for therapy of a pulmonary tuberculosis is inexpedient.

To children Ronkoleykin intravenously kapelno according to the schemes similar to schemes is recommended to apply to adults. For cultivation use chloride sodium solution for injections of isotonic 0,9%.

Single dose of drug on the volume of required isotonic solution for children (depending on age):

• The neonatality period (up to 1 month): 0,1 mg on 30-50 ml;
• 1-12 months: 0,125 mg on 100 ml;
• 1-7 years: 0,25 mg on 200 ml;
• 7-14 years: 0,5 mg on 200 ml;
• Of 14 years: 0,5 mg on 400 ml.

Side effects

In the course of Ronkoleykin's introduction the short-term fever and temperature increase can appear. Such condition of the patient is stopped by ordinary therapeutic drugs and cannot be the cause for the termination of administration of solution, and also interruption of a course of therapy. Hypodermic administration of medicine can be followed by the following reactions in the place of an injection: morbidity, reddening, consolidation.

The overdose is possible at introduction of a single dose of Ronkoleykin more than 7 mg, it is expressed as hypotonia, disturbance of a cordial rhythm, fever, dermatological allergic reactions. After drug withdrawal these side effects are stopped. It is in case of need recommended to carry out symptomatic therapy.

Special instructions

Special researches regarding influence of medicine on ability to control of difficult mechanisms and to driving of motor transport were not conducted. In this regard patients should refrain from these types of activity at development of undesirable effects from an organ of sight and/or decrease in speed of psychomotor reactions and ability to concentration of attention (up to their permission).

Medicinal interaction

Therapy by Ronkoleykin can be combined with treatment by any other medicines, but it is impossible to mix solution with them rolled into one or the syringe.

Against the background of long use of glucocorticosteroids efficacy of Ronkoleykin can decrease.

Terms and storage conditions

To store in the place unavailable to children, at a temperature of 2-8 °C.

Period of validity – 2 years.

It is possible to transport no more than 10 days, at a temperature of 9-25 °C.
