Kollalizin – kelloidolitichesky means.

Form of release and structure

Dosage form – lyophilisate for preparation of solution for topical and parenteral administration: powder or porous mass of white color (in ampoules, on 10 ampoules in a pack cardboard complete with an ampoule knife or the scarificator).

Active ingredient – a collagenase, in 1 ampoule – 100, 200, 300, 400, 500, 600, 700, 800, 900 or 1000 KU (kollagenazny units).

Indications to use

Treatment and prevention:

• Symblepharon;
• Opacification of a cornea;
• Postburn hems of a conjunctiva;
• Cicatricial changes (keloid cicatrixes) of skin century;
• Retina hems;
• Opacity of the vitreous body;
• Secondary cataract;
• Strictures of slezootvodyashchy ways (the lacrimal tubules and nasolacrimal channel);
• The traumatic vitreous hemorrhage occupying more than 1/2 its volumes (in time of 3 days after formation of a hemophthalmia);
• Plastic iridocyclitis (in a stage of subsiding of the inflammatory phenomena);
• Cicatricial changes of skin (nakozhno).

Contraindications

Absolute:

• Existence of defects in a sclera;
• Symblepharon combination to a helcoma;
• Existence of signs of a blood coagulation in a vitreous;
• Unfinished process of epithelization of a cornea after a burn;
• Vitreous hemorrhage less than 1/2 its volumes;
• Age up to 18 years;
• Pregnancy and lactation;
• Hypersensitivity to a kollalizin (it is shown by sharply expressed reaction after introduction 1 KE).

Route of administration and dosage

Kollalizin apply outwardly, enter podkonjyunktivalno, and also directly into the defeat center (a hem, commissure, a vitreous, etc.) by means of electro-or a fonoforeza.

Before use contents of an ampoule are dissolved in water for injections, solution of novocaine of 0,5% or solution of sodium of chloride of 0,9%.

Before Kollalizin's use check sensitivity of the patient to a collagenase. For this purpose under a conjunctiva of a sore eye enter 1 KE drugs and within 48 hours watch the patient. Treatment is carried out only in case of lack of allergic reaction.

In need of cutaneous use of drug check its portability skin, for this purpose apply solution in a medical dose for 24 and 48 hours.

Schemes of use of Kollalizin:

• Prevention of postburn symblepharon: on 10 KE in solution of novocaine of 0,5% enter into fabrics of the burned arch after full epithelization of a cornea. The subsequent injections do bucketed 2-3 days (that the reactive phenomena on the previous injection abated). The course of treatment consists of 5-7 injections, a total dose – 60-70 KE, therapy duration – 2-3 weeks;
• Treatment of symblepharon, keloid and other cicatrixes century: 100 KE in solution of novocaine of 0,5% enter at once under a conjunctiva into commissure between an eyeglobe and a century or into thickness of a hem. The procedure is carried out, exercising careful visual control by means of a binocular magnifying glass. In 2-5 days, after subsiding of the reactive phenomena, do a repeated injection. The course of treatment consists of 3 injections, a total dose – 300 KE, therapy duration – 3 weeks;
• Treatment of narrowing of slezootvodyashchy ways: 50 KE in 2 ml of solution of novocaine of 0,5% use for washings. The course of treatment consists of 7-10 procedures. If necessary in 2-3 weeks therapy is repeated;
• Treatment of a traumatic vitreous hemorrhage: 1 KE in 0,2 ml of solution of sodium of chloride of 0,9% enter once into the central area of a vitreous through a sclera section in the field of a flat part of a ciliary body;
• Treatment of an opacity of the vitreous body, the formed hemophthalmia, cicatricial process in a retina: on 10 KE enter into 0,2 ml of solution of novocaine of 0,5% daily or every other day subkonjyunktivalno (under an eyeglobe conjunctiva) or every other day retrobulbarno (under a tenonova the capsule). The course of treatment consists of 7-10 injections, a total dose – 70-100 KE. If necessary in 2-3 weeks the course of therapy is repeated;
• Treatment of the fresh growing keloid cicatrixes, hypertrophic hems, the created hems prescription no more than 1 year, and also prevention of formation of cicatricial changes after excision of keloid cicatrixes (after epithelization of a wound surface or a full wound repair) at patients with the increased tendency to excess proliferation of connecting fabric: Kollalizin apply by an electrophoresis method. Aqueous solutions of drug enter every other day from a positive electrode within 20 minutes with use of current 0,03-0,1 мА on ^cm2. The course of treatment includes no more than 15 procedures. In total carry out 2 or 3 courses with 2-month intervals. Kollalizin's doses select individually depending on the size, a stage of development and clinical manifestations of a hem: hypertrophic hems – on 500-1000 KE in 10 ml of injection water, a total dose – 22 500-45 000 KE; the fresh growing keloid cicatrixes created hems by prescription more than 1 year, prevention of formation of cicatricial changes after excision of keloid cicatrixes – on 1000-2000 KE in 10 ml of injection water, a total dose on 3 courses of treatment – 45 000-90 000 KE.

Treatment of opacifications of a cornea, secondary cataract and plastic iridocyclitis (back synechias) carry out one of the next ways:

• Fonoforez: 50 KE in 10 ml of water for injections enter into an eye by means of the IUT 0,88-4-2 sensor of the device "T-5 Ultrasound". Procedure duration – at intensity of 0,2 W / see the Course of treatment includes 5 minutes 10 procedures. If necessary in 2-3 weeks conduct a repeated course;
• Electrophoresis: 50 KE in 10 ml of water for injections enter into an eye from a positive pole, using a current to 1 мА. One procedure lasts up to 10 minutes. The general course of treatment consists of 10 procedures. If necessary in 2-3 weeks conduct a repeated course;
• Elektrofonoforez: during a fonoforez connect an electrophoresis. Duration of procedures – 5 and up to 10 minutes respectively. If necessary in 2-3 weeks conduct a repeated course;
• Subconjunctival introduction: on 10 KE enter into 0,2 ml of solution of novocaine of 0,5% under an eyeglobe conjunctiva. The course of treatment includes 7-10 injections, the general course dose makes 70-100 KE. If necessary in 2-3 weeks therapy is repeated.

Outwardly Kollalizin is recommended to apply to prevention of a hypertrophy of a hem to patients after the dermabrasion which is carried out concerning removal of hems or mimic wrinkles of the person and also to prevention of formation of cicatricial educations after carrying out electrothermic coagulation concerning various new growths of skin. Treatment is begun in the postoperative period after epithelization of a wound surface or a full wound repair. Drug is applied on a hem on 500 KE in 10 ml of injection water daily within 4-5 weeks. The total dose makes 15 000-30 000 KE.

Side effects

Kollalizin can cause allergic reactions. Formation of helcomas is also possible.

Special instructions

It is possible to avoid side effects if to begin treatment after the termination of epithelization of a cornea and receiving a negative sensitivity test to drug.

At development of allergic reactions Kollalizin cancel and carry out antiallergic therapy: local influence by cold, instillations of corticosteroids, purpose of drugs of calcium and antihistamines (Dimedrol or Suprastinum).

In case of a helcoma use of drug should be stopped immediately, to carry out antibacterial therapy and to appoint the means stimulating epithelization.

Medicinal interaction

Heavy metals, antiseptic agents, detergents and soaps weaken action of a collagenase.

Kollalizin it is not necessary to apply along with tetracyclines, gramicidin and tyrothricin.

Terms and storage conditions

To store in the place unavailable to children protected from light and moisture. To observe temperature condition 4-8 ºС.

Period of validity – 2 years.
