Digitoxin

Digitoxin – the low-polar cardiac glycoside received from different types of a foxglove.

Structure and form of release

Is issued in the form of candles on 0,15 mg for topical administration and in the form of tablets on 0,1 mg for internal use.

Active ingredient – digitoxin.

Digitoxin issue conditions – according to the recipe.

Pharmacological action

Has the expressed cardiotonic effect (increases force of cordial reductions), slows down heart rate. Has strong cumulative properties, from digestive tract it is soaked up quickly.

Drug begins to work 2-4 hours later after reception. Collects in an organism.

Indications to Digitoxin use

The tablets Digitoxin according to the instruction are appointed at:

• to severe and moderate form of heart failure, in particular, at low cordial emission with a normal sinoatrial rate and existence of a cantering rhythm, at dilatation of ventricles;
• chronic heart failure with a takhisistolichesky form of atrial fibrillation;
• for the purpose of decrease in ChSS at sinus tachycardia owing to an acute heart failure, a paroxysm;
• paroxysmal supraventricular arrhythmia (atrial flutter, ciliary arrhythmia, supraventricular tachycardia).

Candles apply Digitoxin according to the recipe in ophthalmology at:

• asthenopia (it is necessary to exclude probability of the wrong selection of points and organic lesion of eyes);
• fatigue of eyes at disturbance of blood circulation, the raised visual loading, eye pains, emergence of attacks of migraine, at the beginning presbyopy.

Ways of reception and dosage

The dosage is selected by individually attending physician, depending on the scheme of treatment and indications.

The dosage of tablets of Digitoxin according to the recipe for adults makes in the first days of 6-12 tablets a day in four receptions each six hours. After achievement of therapeutic effect the dosage decreases to 1 tablet one-two times a day.

Prescribe patients with digestive disturbances or stagnation candles in system of a portal vein Digitoxin according to the instruction. The drug is administered in a rectum on 1 or 2 candles two times a day the first few days then the dosage decreases to 1 candle one-two times a day.

Contraindications

Tablets and candles of Digitoxin according to the instruction are contraindicated at:

• hypersensitivity to drug;
• abnormal liver function;
• glikozidny intoxication

Digitoxin tablets according to the recipe are prohibited at:

• To AV blockade of I and II degrees;
• the expressed bradycardia;
• acute myocardial infarction;
• the isolated mitral stenosis;
• to cardiac tamponade;
• ventricular tachycardia;
• premature ventricular contraction;
• WPW syndrome;
• unstable stenocardia.

Side effects

Use of Digitoxin according to the instruction can cause such by-effects:

• Cardiovascular system: disturbance of a cordial rhythm, AV blockade, bradycardia;
• Central nervous system: euphoria, the increased fatigue, a depression, a syncopal condition, a delirious state, a sleep disorder, dizziness, a headache, confusion of consciousness;
• Alimentary system: diarrhea, nausea, lack of appetite, vomiting;
• System of a hemopoiesis: thrombosis of mezenteralny vessels;
• Organs of sight: front sights before eyes, decrease in visual acuity, a xanthopsia, scotomas;
• Reproductive system: at long treatment the gynecomastia can develop.

Interaction of Digitoxin with medicines

Use of Digitoxin according to the recipe in combination with magnesium - and aluminum-bearing antacids cause some reduction of its absorption.

Simultaneous use of Digitoxin according to the instruction with GKS is fraught with development of glikozidny intoxication.

In combination with azithromycin substantial increase of concentration of active ingredient in blood is observed that leads to development of glikozidny intoxication.

The concomitant use of Digoxin according to the recipe with aminglutetimidy, Amiodaronum and Amphotericinum In can cause glikozidny intoxication.

The concomitant use of drug with barbiturates causes acceleration of metabolism of digitoxin and its transformation into digoxin.

The digitoxin combination with diltiazem and verapamil causes increase in concentration in blood of active ingredient.

In combination with dozipiramidy heavy arrhythmia can develop.

Simultaneous use with phenylbutazone, rifampicin, Phenytoinum, Spironolactonum reduces concentration of active ingredient in a blood plasma.

The drug combination with karbenoksolony as it causes reduction of potassium in serum is not recommended, liquid in an organism detains and increases arterial pressure.
