Emla – the local anesthetizing drug intended for surface anesthesia.

It is presented in a dosage form of cream for external and topical administration as homogeneous mass of white color. The main active ingredients of drug of Eml are local anesthetics of amide type – lidocaine (25 mg) and прилокаин (25 mg). Treat excipients of medicine a macrogoal глицерилгидроксистеарат, carbomer 974P, sodium hydroxide and the purified water.

Pharmacological action of Eml

The medical effect of drug of Eml according to the instruction is reached due to blocking a voltage - dependent Na + - channels that interferes with emergence of impulses in nerve terminations and to their carrying out on nerve fibrils. Active ingredients of Eml oppress carrying out not only painful impulses, but also impulses of other modality.

The maximum expressiveness of anesthesia after putting cream of Eml and removal of an occlusive bandage comes in 1 hour and about 30 minutes last.

The mucous membrane of generative organs is exposed to anesthesia quicker, than intact skin – due to the accelerated drug absorption. In 5-10 minutes after putting drug of Eml on generative organs at women anesthesia which duration makes 15-20 minutes is reached: it is sufficient for stopping of the pain syndrome connected with use of the argon laser.

Degree of anesthetizing Eml's effect depends on duration of application of drug and its dosage.

In responses to Emla the negative impact of cream on a bacterial flora of bodies and process of healing of ulcers is noted.

Indications to Eml's use

Eml's cream and analogs of drug are successfully applied in need of implementation of surface anesthesia of mucous membranes and integuments, carrying out surgeries on integuments and mucous membranes of superficial character, capture of rags of skin in surgical practice or Combustiology, before introduction to needle skin to catheterization of blood vessels or blood sampling, before excision and mechanical clarification of trophic ulcers.

Besides, Eml's use allows to achieve the necessary anesthetizing effect during removal of tattoos.

Contraindications

In the instruction to Emla the following contraindications to drug use are noted:

• Idiopathic or inborn methemoglobinemia;
• Hypersensitivity to local anesthetics of amide structure;
• Children's prematurity up to 37 weeks;
• Children up to 12 months;
• Anesthesia before introduction of live vaccines.

With care use of drug of Eml and analogs of means the persons having atopic dermatitis is recommended.

Side effects of Eml

In spite of the fact that Emla, according to reviews to drug, is had well, emergence of the following side effects is in certain cases possible:

• Hyperemia;
• Hypostases;
• Burning;
• Itch;
• Allergic reactions, including, acute anaphylaxis.

At overdose of medicine excitement of the central nervous system, a methemoglobinemia, a coma, spasms can be shown. Treatment of this state is performed by means of symptomatic therapy: maintenance of the vital functions, lung ventilation, anticonvulsant therapy. In case of a methemoglobinemia it is recommended to enter solution of methylene blue slowly.

Eml route of administration

According to Eml's instruction it is put outwardly, under the occlusive bandage which is available in packaging, not less than in 1 hour prior to holding a procedure. In medical practice on a bandage the exact time of putting drug registers. After its removal the remains of cream wipe ethanol.

For skin anesthesia (for adults) Eml's cream is applied calculated on the basis of 1,5 g / 10 to sq. cm, leaving a bandage for 1-5 hours.

The dosage for children of 3-11 months makes 1 g / 20 sq. cm, children 1 years – 1 g / 10 are more senior than quarter see.

For the purpose of anesthesia of generative organs 5-10 g of cream of Eml for 5-10 minutes, without the need for overlaying of a dense bandage are put.

At the diagnosis drug application duration Emla should not exceed atopic dermatitis 30 minutes.

Special instructions

Eml's cream and analogs of drug is not recommended to be applied on open wounds, except for trophic ulcers of the lower extremities.

It is necessary to be careful when drawing Eml's on skin around eyes: loss of protective reflexes can provoke damage of a cornea. At hit of cream in eyes it is necessary to wash out immediately the angry places normal saline solution or water and to apply a protective bandage eyes.

Responses to Emla do not recommend to apply drug on the injured tympanic membrane because of possible risk of penetration of active ingredient into a middle ear.

Because of lack of the conducted researches and a lack of data on the speed of absorption of drug it is not necessary to use Eml's cream on mucous membranes of generative organs at children.

Decrease in reaction at management of transport and equipment when using cream of Eml was not observed.

Eml storage conditions

Storage of drug of Eml is forbidden at a temperature above 30 degrees Celsius and medicine freezing. To protect from children!

Period of validity – 3 years. Use of drug after its expiration is not reasonable.
