Active ingredient of Novocaine – Procainum a hydrochloride:

1 ml of solution for injections: 2,5 mg, 5 mg, 10 mg or 20 mg;
1 ml of solution for infusions: 2,5 mg or 5 mg;
1 suppository: 100 mg.

Auxiliary components:

Solution for injections: solution of Acidum hydrochloricum of 1 M, water for injections;
Suppositories: solid fat.

Indications to use

Use of Novocaine in the form of solutions for injections and infusions:

Blockade: vagosympathetic, perinephric;
Anesthesia: conduction, infiltration, spinal, epidural;
Pain syndrome at stomach ulcer and a duodenum;
Nausea;
Hemorrhoids.

Suppositories rectal use at anal fissures and hemorrhoids as anesthetic.

Contraindications

Age up to 18 years;
Hypersensitivity to paraaminobenzoic acid and other Anesthetics Ether (local).

It is necessary to be careful when using drug in immediate surgeries with acute blood loss, at patients with the renal failure, chronic heart failure, diseases of a liver and other pathologies which are followed by decrease in a hepatic blood-groove, progressing of cardiovascular insufficiency (usually against the background of development of a heart block, shock), a proctitis, at deficit of pseudo-cholinesterase, at the weakened patients, elderly people (65 years are more senior), seriously ill patients.

Use during pregnancy and breastfeeding is shown only if the estimated advantage for mother exceeds potential harm for a fruit or the child.

Route of administration and dosage

Solution for injections: enter intravenously (in/in), intramusculary (in oil), vnutrikozhno (boiled and smoked) or accept inside. The recommended dosing: in/in – 0,5% solution on 1-15 ml, having mixed with isotonic solution of sodium of chloride; in oil – 2% solution on 5 ml a day 3 times a week, a course of treatment makes 12 injections (no more than 4 courses a year); boiled and smoked – 0,5% appoint solution, a dose individually, use at neurodermatitis, eczemas, a sciatica for juxtaspinal and circular blockade; inside – 0,25% or 0,5% solution on 30-50 ml 2-3 times a day;
Solution for infusions: enter intravenously, the mode of dosing is appointed by the doctor on the basis of clinical indications, individually;
Suppositories rectal: enter deeply into an anus after spontaneous bowel emptying or a cleansing enema. The recommended dosing: on 1 suppository 1-2 times a day. Course of treatment no more than 5 days. In the absence of the anesthetizing effect it is necessary to see a doctor.

Side effects

Cardiovascular system: thorax pain, peripheral vazodilatation, decrease or increase in the arterial pressure (AP), bradycardia, arrhythmias, collapse;
Nervous system: drowsiness, a headache, weakness, dizziness, a tremor, motive concern, spasms, a loss of consciousness, a lockjaw, acoustical and visual disturbances, a syndrome of a horse tail (paresthesias, paralysis of legs), a nystagmus, paralysis of respiratory muscles (is more often at subarachnoidal anesthesia), disorder of motor and touch conductivity;
Urinary system: involuntary urination;
System of a hemopoiesis: methemoglobinemia;
Alimentary system: involuntary defecation, nausea, vomiting;
Allergic reactions: a skin itch, rash, a small tortoiseshell (on mucous membranes and skin), anaphylactic reactions, an acute anaphylaxis, weakness, dizziness, decrease in the ABP.

Besides, use of suppositories rectal can cause side effects:

Local reactions: during the first days of use – passing desires to defecation and feeling of discomfort (do not demand drug withdrawal);
Seldom: against the background of use of high doses – an itch and a hyperemia in an anus.

Special instructions

Treatment has to be followed by careful control of functions of respiratory, cardiovascular and nervous systems.

Administration of local anesthetic can be made in 10 days after phase-out of monoamine oxidase inhibitors.

Patients need to observe extra care at control of vehicles and mechanisms.

Medicinal interaction

Novocaine strengthens the oppressing action on a nervous system of other medicines.

Simultaneous use with warfarin, daltepariny sodium, ardepariny sodium, danaparoidy sodium, heparin, enoksapariny sodium increases risk of development of bleedings.

At a combination to monoamine oxidase inhibitors (Procarbazinum, furasolidone, селегилин) the probability of decrease in the ABP increases.

Vasoconstrictors, including Epinephrinum, Phenylephrinum, метоксамин promote lengthening of the period of mestnoanesteziruyushchy effect.

As action of Procainum (especially at use of high doses) reduces anti-myasthenic action of antikholinesterazny means, correction of the mode of dosing at treatment of a myasthenia is required.

Metabolism of Novocaine is reduced by cholinesterase inhibitors (cyclophosphamide, anti-myasthenic means, a demekariya bromide, Thiotepum, an ecothiostalemate iodide).

Paraaminobenzoic acid (Procainum metabolite) is an antagonist of sulfanamide drugs therefore at simultaneous use their bactericidal action decreases.
