Anafranil

Anafranil – drug of antidepressive action, has the expressed sedative and moderate analgeziruyushchy effect.

Form of release and structure

• Tablets, coated (on 10 pieces in blisters, in a cardboard pack 2 or 3 blisters);
• Solution for intramuscular and intravenous administration (in ampoules on 2 ml, in a cardboard pack of 10 ampoules).

Active ingredient of Anafranil – кломипрамин.

Auxiliary components as a part of tablets: lactose, stearic acid, starch corn, magnesium stearate, silicon dioxide colloid anhydrous, talc, titanium dioxide, glycerin of 85%, vinylpyrrolidone/vinyl acetate copolymer, sucrose crystal, gidroksipropilmetiltsellyuloza, PVP K30, MKTs, ferrous oxide of yellow 5%, titanium dioxide of 95%, polyethyleneglycol 8000.

Excipients as a part of solution: water for injections, глицерол.

Indications to use

• Depressions of neurotic, involutional, endogenous, reactive, organic character;
• The depressive syndrome which developed against the background of schizophrenia;
• Frustration of the personality;
• The depressions provoked by long pain;
• Panic attacks of fear.

Anafranil is effective at a chronic pain syndrome at cancer patients, patients having migraine, rheumatism, post-herpetic neuralgia, a peripheral neuropathy, a narcolepsy and a katalepsy.

Quite often drug is used for prevention of migraine and removal of a headache.

Contraindications

• Heart attack;
• Alcoholic poisoning and drugs;
• Closed-angle glaucoma;
• Heavy disturbances of conductivity;
• Age up to 12 years;
• Pregnancy and lactation;
• Hypersensitivity to drug components.

With care appoint Anafranil at the following states:

• Alcoholism;
• Bronchial asthma;
• Maniac-depressive psychosis, schizophrenia, epilepsy;
• Oppression of the hemopoietic function of marrow;
• Stenocardia, chronic heart failure, hypertensia, arrhythmia, heart block, stroke;
• Disturbances of motility of a GIT, renal and liver failure, thyrotoxicosis, urination delay, prostate hyperplasia;
• Advanced age.

Route of administration and dosage

Accept drug inside, after food or during it, or enter intramusculary or intravenously.

For treatment of phobias, depressions, persuasive states accept Anafranil in 2-3 times a day 25 mg.

For the first week the day dosage of means is increased gradually to 100-150 mg. After the state improves, the patient is transferred to a maintenance therapy – 50-100 mg a day.

Intramusculary the drug is administered in a dosage 25-50 mg then every day increase a dose by 25 mg, bringing it to 100-150 mg/days. After stabilization of a state reduce quantity of pricks, transfer the patient to administration of drug in tablets.

Intravenously 50-75 mg of Anafranil enter for 1,5-3 hours. Single infusion is carried out, before introduction drug is dissolved in Dextrosum or solution of sodium of chloride. After the necessary effect is reached, administration of drug is continued on an extent of 3-5 days.

As the supporting treatment to the patient appoint Anafranil's use in tablets.

For treatment of a narcolepsy appoint drug in a daily dosage of 25-75 mg, inside.

For removal of chronic pain take a pill in a dosage of 10-150 mg/days.

At attacks of fear accept antidepressant in a dosage of 10 mg/days.

To elderly patients appoint means in a dosage of 10 mg/days, gradually increasing a dose to 30-50 mg.

To children drug appoint 10 mg in a dose and for 10 days increase it:

• For children of 5-7 years – to 20 mg;
• For children of 8-14 years – to 20-50 mg;
• For children 14 years – to 50 mg and are more senior above.

Side effects

Anafranil's use can cause such side effects as drowsiness, concern, uneasiness, aggression, a memory impairment and concentration of attention, nightmares, psychosis, a sleep disorder, a sonitus, hallucinations, irritability, an indisposition, lability.

Besides, drug is in certain cases capable to provoke a tremor, deterioration in sight, tachycardia, dryness in a mouth, a lock, the increased sweating, arrhythmia, an ataxy, a collapse, hypotension, nausea and vomiting, hepatitis, heartburn.

At intravenous administration of Anafranil thrombophlebitis can develop, appear burning, a lymphangitis, rash on skin.

At overdose of drug there are a drowsiness, an ataxy, sleeplessness, concern, confusion of consciousness, a stupor, muscular rigidity, epileptic attacks, tachycardia, disturbances of endocardiac conductivity: in rare instances – a cardiac standstill.

Also Anafranil's use in high doses can cause short wind, vomiting, a mydriasis, respiratory depression, cyanosis, an oliguria, the increased sweating, an anury.

At oral administration of drug for treatment of overdose wash out a stomach, accept the coal activated. In hard cases, at arrhythmia, pressure decline, a coma enter cholinesterase inhibitors, carry out artificial ventilation of the lungs, anticonvulsant therapy. The diuresis and a hemodialysis are considered as inefficient.

Special instructions

Before an initiation of treatment Anafranil it is necessary to eliminate a hypopotassemia.

In the presence of liver diseases during therapy of drug it is necessary to control activity of liver enzymes.

The good effect gives Anafranil's combination to benzodiazepines. At the same time throughout treatment the drug dosage is gradually increased (depending on portability), and benzodiazepine is cancelled. It is desirable that treatment lasted not less than half a year.

At treatment by Anafranil it is necessary to avoid driving of motor transport and control of potentially dangerous mechanisms which demand high concentration of attention.

It is necessary to cancel drug gradually (in order to avoid side reactions).

Analogs

Analogs of means are Anafranil SR, Klominal, Klofranil and Klomipramin.

Terms and storage conditions

To store in the dark place unavailable to children, at a temperature not above 30 °C.

Period of validity – 5 years.
